Evening Maktab Program Application Form

EVENING MAKTAB FEE STRUCTURE

Masjid-ut-Taqwa Evening Maktab Program Registration is now open. In Sha Allah, classes will be held from Monday to Thursday from 06:00PM to 8:00PM.

All payments will be collected through monthly Pre-Authorized Debit (PAD). Families registering for the first time are required to submit a valid void cheque as part of this form.

The evening maktab fee is $40 per child/month.

Fee Breakdown:

1 child – $40/month
2 children – $80/month
3 children – $115/month

Note: The responsibility for student drop-off and pick-up lies solely with the parents or guardians. Masjid-ut-Taqwa is not liable for any accidents, injuries, or damages that may occur during participation in evening maktab programs or within the facility where classes are held.

To set up direct debit payments, please follow the instructions below:

Upload a PDF of your bank account information (also known as a “void cheque”).
This PDF can be downloaded directly from your online banking.
Attach the file in the section below of the registration form.
A physical cheque is not required.

If you do not use online banking, you may instead:

Upload a picture of a physical cheque clearly marked “VOID”, or
Provide a picture of a printed Direct Debit Account Information form obtained from your bank branch.
